Home Health & Physical Therapy Arrangements
We arrange for in home physical therapy, wound care, nurse visits and more at the comfort of your home.
Speciality Referrals and DME orders
We partner with dermatology, podiatry, and audiology services to care for our patients needs. We also help co-ordinate with orders for wheelchairs, walkers, hospital beds and other medical supplies.
Mobile Lab and Diagnostic Imaging orders
We offer mobile lab and radiology serives through trusted local contracting providers. We work with them seemlessly to attain the results and follow up with our patients in a timely manner.
Hospital and ER Follow-ups
Although our exceptional care minimizes hospital visits, we do make sure to follow-up our home bound patients within 10 days of discharge to help manage their mediations and guide them for a healthy recovery.
Medication Management
The physician will routinely evaluate your medication list for accuracy and remove or reduce unnecessary medications by evaluating the benefit vs risk of such medication regimen.
House Calls
Your convenience and safety is of utmost importance to us. We will try to visit you in any setting if you can't make it to our office. We always ask "What do our patients need? How can we be of assistance to them?"
